• In order to be valid, the healthcare directives document must be executed via one of the following procedures:

        a) Before a notary.

        b) Before three witnesses: (at least two of the witnesses must not be related to the executor via kinship closer than the second degree or linked to the executor as a result of shared estate).

• The Government of Navarre provides a model for registration requests.

• The request must include the following documents:

        1. The document that the individual wishes to register in the Advance Directives Registry of Navarre.

        2.Certified copies of the national identification documents or passports of the executor and each of the witnesses.

In the event that the advance directives document has been executed before a notary, an authorised copy must also be included.
